Our
PROGRAM
The Long-Term Care Ombudsman Program is authorized by the Federal Older Americans Act. This act requires every state, through the Office on Aging, to create a statewide ombudsman program to “investigate and resolve complaints made by, or on behalf of, older individuals who are residents of long-term care facilities” (including nursing homes, assisted living and board care facilities).
“Ombudsman” is a Swedish word meaning citizen representative. A Long-Term Care Ombudsman serves as an advocate who seeks to resolve complaints made by, or on behalf of, people who are consumers of Long-Term Care services.

To Advocate for the Rights and Well-Being of Long-Term Care Facility Residents, Providing Support, Resolving Concerns and Facilitating Access to Quality Care and Services.
ADVOCACY
We advocate for resident rights including freedom from abuse and the right to express grievances without fear of retaliation.
EMPOWERMENT
We empower residents and/or family members to advocate for themselves or their loved one.
CONNECTION
We connect trained volunteer ombudsmen with long-term care facilities in nine counties throughout Northeast Indiana.
